How Do I Sign Up?

Form and registration fee may be dropped off at the church office anytime between 9:00 a.m. and 3:00 p.m., Monday through Thursday. Forms may also be downloaded below.

2011-2012 Upward Basketball Registration

Registration

The registration cost per child for basketball is $48.
Deadline for registration is November 30.
Scholarships are available.

Evaluations

Everyone must attend one basketball evaluation.
They will take place at Morning Star Friends Church as follows:
1st through 8th Grade Boys/Girls

Saturday, November 26, between 9:00 a.m. and 6:00 p.m.
Monday, November 28, between 6:00 p.m. and 9:00 p.m.
Wednesday, November 30, between 6:00 p.m. and 9:00 p.m.
